I do not bill insurance directly; however, I can provide you with a superbill that you may submit to your insurance for potential reimbursement. Please note that I do not participate with Medicare, Medicaid, personal injury cases, or any other healthcare insurance plans.
If you have insurance and wish to use it for treatment, please arrange reimbursement directly with your provider. I encourage you to contact your insurance company before starting treatment to understand the terms and level of coverage available to you.

allow 60-75 minutes for your first visit($175.00)
20 minutes per Visit for Follow-up ($60).
